Your Appointment

On your first appointment, we will begin a permanent record. Please bring any records from the referring physcian or from your previous health care provider. We ask that you bring your current insurance information at the time of the appointment as well. Prior to your examination and consultation with the practitioner, a nurse will ask you for a complete medical history.

Making Appointments

You can reach our receptionists to schedule an appointment between 8:30 am and 5:00 pm by calling our main office phone number: 908-782-2825. If you need to call after hours, our answering service will be able to direct your call if it is an emergency or they will ensure we receive the message the following business day.

Billing

We prefer not to send bills, since the costs of postage and paperwork affect our fees. We will appreciate your payment at the time of your visit. Some insurances cover routine preventative care at 100% and no co-pay is due for these visits. If you are returning for a problem, consultation, or follow-up appointment there will be a co-pay due since these visits are not considered part of preventative care.

We accept VISA, MASTERCARD, checks, and cash. If you would like to arrange a payment plan, please ask to speak to a billing representative.

Insurance

We participate with most insurances. Contact your insurance provider to check that we are in your network and if  you require a referral. If you have any further questions, please call our Billing office. We would be happy to answer your questions.

WE DO NOT PARTICIPATE WITH MEDICAID, HORIZON NJ HEALTH, UNITED HEALTHCARE COMMUNITY PLAN, AMERIGROUP OR ANY OTHER STATE ASSISTANCE PLAN.

 

The patient is responsible for knowing the benefits and restrictions of the insurance policy and should verify coverage before doing any additional testing suggested by the healthcare provider. If these services require a referral, kindly give our office 48 hour notice. You must bring your insurance card with you at every visit to ensure we have correct information. Insurance plans vary and cover visits differently, you are responsible for any amount not covered by your plan.

Confidentiality

Your medical record is strictly private. We will not give out information regarding your care to your employer, friends, or relatives without your permission.

Cancelling/Rescheduling Appointments

If you are unable to keep your appointment or running late, please call the office as soon as possible. This courtesy allows us to be of service to other patients.
